DATA SUBJECT APPLICATION FORM

PURSUANT TO THE PERSONAL DATA PROTECTION LAW NO. 6698

The Law on Protection of Personal Data No. 6698 (hereinafter referred to as “LPPD”) provides for the following rights to personal data owners referred to as “data subjects” (hereinafter referred to as “Applicant”) under Article 11 thereof:

● To learn whether or not your personal data has been processed;

● To request information regarding the processing of your personal data, if your personal data has been processed;

● To learn the processing purpose of your personal data and whether such data is used in accordance with the intended purpose;

● To learn about the third parties at home or abroad to whom your personal data has been transferred;

● To request rectification in case your personal data has been processed incompletely or inaccurately and to request notification of such rectification to the third parties to whom your personal data has been transferred;

● To request the erasure or destruction of your personal data under even though personal data has been processed in accordance with LPPD and provisions of other relevant laws, if grounds for such processing no longer apply and to request notification of such erasure or destruction to the third parties to whom your personal data has been transferred;

● To object to occurrence of any result that is to your detriment by means of analysis of processed data exclusively through automated systems;

● To request compensation for the damages in case you suffer any damages due to unlawful processing of your personal data.

Pursuant to paragraph 1, Article 1 of the LPDD and the Communique on Procedures and Principles of Application to Data Controller, applications to our Company in relation to these rights must be made in writing, by sending an electronic mail to kvkk@bvnair.com using an electronic mail address registered in our Company’s system, or other methods that may be determined by the Personal Data Protection Board (hereinafter referred to as the “Board”).

In this context, applications to our Company “in writing” can be made by printing out the Data Subject Application Form and:

● By applying to our Company personally,

● Via a notary public or registered mail,

● If the applicant has an e-mail address registered in our Company's systems, by sending an e-mail to our Company at kvkk@bvnair.com using the registered e-mail,

Below you can find information about how to submit written applications to our Company via specific written application channels:

You will receive a response regarding your application within thirty days from the date of receipt of the application, depending on the nature of the request, pursuant to paragraph 2, Article 13 of the LPPD. Our response will be delivered to you in writing or electronically pursuant to the provision under Article 13 of the LPPD.
